In order to participate in this program, you must be taking STELARA® for plaque psoriasis or psoriatic arthritis under the supervision of a physician. Not all stories will appear in patient educational materials. Stories will be evaluated and selected on an individual basis. Please note, you will not be compensated for submission or evaluation of your story.
The information you provide will only be used by Janssen Biotech, Inc., our affiliates, and our service providers to fulfill your request to contact you regarding your interest in participating in the STELARA® Stories Program and any optional request you may indicate.
